6.2 Patient Safety
AHP strives to serve as a critical link for providers to access patient safety information and tools to improve the safety in their practices.
For Facilities, the following resource is available to assist in assessing the Patient Safety Culture and Status. This tool is valuable in determining the base measure of patient safety in the facility as well as the culture of patient safety. The tool, as described below, is available at no cost:
- Patient Safety Self-Assessment Tool: This tool is a survey to determine if safety practices are in place at your facility. Visit the Institute for Healthcare Improvement website for more information.
