Ancillary Products

Avera Health Plans provides a variety of products to meet the needs of small and large businesses, no matter the size.

Depending on your needs and budget, you can add services to your benefits package in a number of ways. We offer defined contribution, bundled packages and stand-alone products.

Defined Contribution

Deciding which benefits to offer your employees doesn't have to rest all on your shoulders. Defined contribution gives you the ability to set a fixed budget for health insurance expenses with the opportunity for employees to select what's important to them from a suite of ancillary products, including life insurance, short-term disability and dental.

Bundled Products

Bundled services are available in a range of low-, medium- and high-priced ancillary benefit packages depending on your needs and budget. Employers choose which services they want to offer employees for a specific cost per month in addition to their premium. Bundled options are available upon request.

Stand-Alone Products

Small and large employers can pick and choose stand-alone services for their benefits portfolio according to needs and budgets. Available options include:

    Life and AD&D

    By adding term or supplemental term life insurance from our partner carriers, employers can give employees the peace of mind that comes from knowing their loved ones are protected. Employers also get a valuable corporate tax deduction and easy to use services, because we handle the plan administration.

    Talk with your employer clients about tailoring basic coverage to fit the needs of their employee group. Most group term insurance policies include an amount of AD&D insurance equal to the amount of life insurance. A benefit schedule may be based on a:

    • Fixed dollar amount
    • Job classification
    • Salary multiplier

    You'll also want to discuss Waiver of Premium Benefits, an option available if an employee becomes totally disabled before age 60 and remains disabled for six months.

    Additional life insurance benefit options include:

    • Accelerated Death Benefit - For terminally ill employees with life expectancy of 12 months or less; pays a portion of life insurance benefit while employee is still living
      • NOTE: This benefit may create taxable income or may adversely affect employee eligibility for Medicaid or other government benefits or entitlements; anyone thinking of filing for accelerated death benefit should obtain professional tax counsel first
    • Extension Coverage, Conversion Coverage - Options that become effective when an employee is no longer working due to disability

    Dental Insurance

    Dental insurance - one of the most desired benefits - gives employers a way to provide employees with access to regular preventive care, such as teeth cleanings and fluoride treatments, as well as other dental services. All of our dental plans offer the convenience of payroll deduction for premiums and the freedom of provider choice.

    Vision Insurance

    Vision insurance gives employees the opportunity to get checkups and prescriptions/lenses. Employees pay a co-pay for office visits. Frames with lens discounts are available.

    • Available to companies with 10+ employees
    • Exams, Frames and Lenses
    • EyeMed network, with access to more than 85,000 vision care providers at almost 27,000 locations nationwide.

    Vision insurance can also be available as a voluntary product available as an option to round out a benefit package. Employers can make this benefit available and only the employee pays.

    Short- and Long-Term Disability

    We offer small and large employers short- and long-term disability options for employees who are unable to work for a period of time due to a disability. Employees can opt to receive payment either as a percentage of salary or in a lump sum amount each week. Disability insurance is sold employee-voluntary or employer-provided.

    Our long-term disability plan options are for two or five years. Our plan pays 60 percent of an employee's earnings, up to a maximum monthly benefit of $6,000.

    Our short-term disability plan options are for 90 or 180 days. These plans replace 60 percent of an employee's weekly earnings, up to a maximum benefit of $1,000 per week.

    Critical Illness

    Protecting hard-earned income is important for those who are managing a critical illness. Critical illness insurance is an option for members who are looking for a security line of income if a critical illness occurs. Insured members would get a payment in a per-day or lump sum benefit to help with expenses.

    • Helps bridge gaps in coverage - can add additional financial support while supplementing existing coverage
    • Cash benefits help protect wallet and savings
    • Flexible spending dollars - broad product that provides protection for a variety of covered conditions, including heart attack, stroke, bypass surgery and more.

    Employee Assistance Program

    Employers can offer their employees counseling services through the Avera Employee Assistance Program. Members, spouses and dependent children receive three confidential counseling sessions every year at no cost. Additional services include:

    • Substance abuse prevention
    • Leadership/manager training and development
    • Consultation
    • Executive coaching
    • Critical incident response

    Flexible Spending Account/Health Savings Account

    There are different ways to help employees budget and pay for medical expenses throughout the year - a Flexible Spending Account (FSA) or a Health Savings Account (HSA).

    An FSA is a tax-favorable benefit option that gives members the option to set aside funds for unreimbursed medical expenses and dependent care expenses. Employers can offer FSA accounts to help pay for qualified medical, prescription, dental and vision expenses.

    If you offer employees a high-deductible health plan, they can pair it with an HSA. This tax-favorable benefit option works much like an FSA but allows them to save money year-over-year.

    Well-Being Services

    Our LiveNOW well-being solution offers a comprehensive umbrella of engagement opportunities and support resources for employers and employees to experience well-being in the workplace.

    The LiveNOW well-being package comes with various member engagement offerings – from health risk assessments and health screenings to health coaching - all accessible from your desktop or mobile phone. Additional services are available for purchase.

    Our LiveNOW team of health coaches and well-being specialists will work with you to identify the best practice approaches to implementing a work site well-being program that suits your company’s health.

    Services include:
