Gold Plans

Gold Plans feature lower deductibles, resulting in higher monthly premiums.

Silver Plans

Cost-share reductions (extra savings) can be used on Silver plans only to lower the amount you pay for deductibles, co-pays and coinsurance.

Bronze Plans

Bronze Plans feature higher deductibles, resulting in lower monthly premiums.

  • Avera 5000 - Direct Only (not available on healthcare.gov)
  • Avera 5200 - Direct Only - High Deductible Health Plan that can be paired with a Health Savings Account
  • Avera 5500 Preferred - for individuals living in Brown, Minnehaha and Lincoln counties
  • Avera 6500 - Direct Only - High Deductible Health Plan that can be paired with a Health Savings Account
  • Avera 6750 - High Deductible Health Plan that can be paired with a Health Savings Account
Catastrophic Plan

To qualify for this plan, you must be under the age of 30 before Jan. 1, 2019, or qualify for a federal hardship exemption.

Documents Needed to Enroll

For each applicant - individual or family member - who will be covered by the policy, provide the following information:

  • Name
  • Birth date
  • Social Security number
  • Address
  • Phone number

If you apply in person or through an agent, bring your previous year's tax return, because it will usually contain this and other important information needed for your application.
