Gold Plans
Silver Plans

Cost-share reductions (extra savings) can be used on Silver plans only to lower the amount you pay for deductibles, co-pays and coinsurance.

  • Avera 5500
  • Avera 6000 - High Deductible Health Plan that can be paired with a Health Savings Account - Direct Only (not available on healthcare.gov)
  • Avera 6250 - Only available for Stanley County
  • Avera 6550 - High Deductible Health Plan that can be paired with a Health Savings Account
Catastrophic Plan

To qualify for this plan, you must be under the age of 30 before January 1 or qualify for a federal hardship exemption.

Documents Needed to Enroll

For each applicant - individual or family member - who will be covered by the policy, provide the following information:

  • Name
  • Birth date
  • Social Security number
  • Address
  • Phone number

If you apply in person or through an agent, bring your previous year's tax return, because it will usually contain this and other important information needed for your application.
