What You Need to Know Before You Apply

Use this checklist to gather what you need before applying for coverage.

  • Home and/or mailing addresses for everyone applying for coverage
  • Social Security Number(s) for everyone applying for coverage
  • Document information for legal immigrations
  • Employer and income information for every member of your household (For example: pay stubs or W-2 forms—Wage and Tax Statements)
  • Employer Coverage Tool Form – required for every job-based plan you or someone in your household is eligible for, even if you don’t enroll
  • An estimate of your annual household income
  • Policy numbers for any current health insurance plans covering members of your household
  • Notices from your current plan that include your plan ID (if you have or had health coverage in the last year)

Need to reset your password?

If you created a Marketplace account for a previous year and it’s been a while since you logged into your account, you may have to reset your password.

  • Select “Forgot your password?” on the Marketplace log in page.
  • You’ll receive a “Forgot Marketplace Password” email. (If you don’t get it, be sure to check your spam/junk mail folder.)
  • Follow the directions to create a new password for your account.

Note: When picking a new password, be sure you don’t use an old password. If you do, you’ll get an error message saying the information you entered is invalid.

If your account is locked, it may take a few hours to unlock it. For best results, wait at least one hour after getting a password reset before logging in.
