Coming Soon: Avera MyChart

The Avera Health Plans member portal is transitioning to Avera MyChart on May 30. MyChart gives you one secure place to view Avera health information and health insurance member benefits together.

On May 30, you can set up your new portal account online and download the Avera MyChart app from the Apple App Store or Google Play. You will need to create a new username and password for MyChart.

Here are a few things you need to know:

  • You will get a new member ID number and new prescription ID numbers. Your ID card will be available digitally, and only the subscriber’s name will be on the card.
  • Plan and benefit access will be automatically established in your new MyChart portal.
  • Proxy will automatically be established for dependent plan members, but you must reestablish proxy access with Avera Health if you also manage a child’s or loved one’s health records.
