Our New Member Portal is Here

Avera MyChart gives you one secure place to view your health insurance member benefits together with your Avera health information (if you're an Avera patient). MyChart replaced the current Avera Health Plans member portal — and also AveraChart and the MHealth app for patients.

All plan information should now be in your MyChart portal. Members will have view-only access to the legacy portal for a limited time.

You may receive an email or text message from Avera or Avera Health Plans encouraging you to sign up for MyChart.

You will need to create a new username and password for MyChart. Your old portal login information will not transfer to the new portal.

Member and Prescription ID Numbers

You and your dependents will receive one member identification (ID) number with the subscriber’s name listed. Your ID includes important information about your health insurance coverage and benefits. It’s important to keep it with you at all times. Here’s what you need to know about your card and how to access and share it.

  • You got a new member ID and prescription ID number in May.
  • We encourage you to sign up for Avera MyChart, where member IDs will be available in digital format within the portal. Physical cards will not be mailed unless you have a Medicare Supplement plan.
  • The digital ID feature with Avera MyChart makes your card easily accessible within your phone when you download the app. Download it in Apple Store or Google Pay
  • Your new ID will automatically be updated at Avera clinics and pharmacies. For non-Avera clinics and pharmacies, you can show your digital ID card in your app.

Proxy Access

Proxy will automatically be established for dependent plan members, but you must reestablish proxy access with Avera Health if you also manage a child’s or loved one’s health records.

Avera MyChart Common Questions

What if I already have MyChart with another health system?

After you create your Avera MyChart account, you’ll be able to link other MyChart accounts, so you can access them from the one app. Learn more about linking accounts.

Will I need to create separate logins for Avera and Avera Health Plans?

No. If you’re an Avera Health Plans member, you’ll be able to use one login to see both Avera Health and insurance information in Avera MyChart. Learn more about the portal and Avera Health Plans.

Can I use a mobile app for MyChart?

Yes, in addition to using the AveraMyChart.org website, you can also download the Avera MyChart app in the App Store and Google Play for Android devices.

APPLE STORE GOOGLE PLAY

If you receive care from an Avera partner location, you can either use the Avera MyChart app or the general MyChart app. However, if you’re an Avera Health Plans member, your insurance coverage details are only available in Avera MyChart.

How is Avera MyChart kept secure?

Keeping your health information private and secure is a top priority at Avera. We use the latest available technology to ensure your data is protected, including two-step verification, secure activation codes, face ID and fingerprint login options for MyChart.

Requiring two-step verification helps ensure your account stays secure even if someone else has your username and password. How it works:

  1. You enter your MyChart username and password on a login screen.
  2. You’ll receive a code via email or text message that includes a single use code.
  3. Follow the steps to complete authentication into your account.

If you requested a verification code via and did not receive it, check your spam or junk folders for your email – and if you have multiple email accounts, check them all.

If you think your email or phone number where you receive Avera MyChart notifications and secure links has been compromised, you can change them in MyChart under Personal Information.
