Your Network and Member ID Cards

MyChart provides secure digital access to your insurance policy information, including digital member ID cards, coverage details, claims and a provider search function to see who’s in your network.

Member ID Card

Your member ID includes important information and will now be stored digitally in your portal and allows you to easily access important insurance and coverage information. You can view and present your health insurance card directly from your mobile device.

To find your member ID number and digital card:

  1. Log in to Avera MyChart.
  2. In the menu, search for or find the Insurance section. Choose Insurance ID Card.

You can show this member ID card at places where you seek care. If you see an Avera provider, this information will automatically update for you.

Please note:

  • Whether it's you logged into MyChart or a dependent using their personal MyChart account, you will only see the subscriber’s name on the member ID card.
  • Only Medicare Supplement members will be mailed a physical card. If you want, you can print a copy of your card by selecting the printer icon when viewing your digital card in MyChart.

Pharmacy ID Numbers

In Avera MyChart, members with pharmacy benefits have access directly into Avera Health Plans RX.

  1. Log in to Avera MyChart.
  2. In the menu, look under the Insurance heading or search for Avera Health Plans Rx.

Finding Providers in Your Network

You can easily search for providers in your area that will be covered by Avera Health Plans. To search for providers, choose Provider Finder under the Insurance section, or use our provider finder search or location finder tool without logging in. Then, you can:

  • Locate providers by specialty, distance from home or work and availability.
  • Review provider education and experience.
  • Find a location near you with providers in your network.
