Who Should Have an Avera MyChart Account
If you’re a current Avera Health Plans Member, you can enroll online at AveraMyChart.org or over the phone. If you’re also an Avera patient, you can also enroll at your next visit.
MyChart Activation Code: Easiest Way to Sign Up
The easiest and fastest way to sign up for MyChart is by using an activation code provided by Avera. Many members and patients will receive their code in a communication from Avera with the MyChart launch, when they have an upcoming appointment or another care touchpoint with us.

- Sign up without a code. Just provide some additional information about yourself, so we can match or build our records. Sign up without activation code.

Why do I need to register for the patient portal?
If you previously used the Avera Health Plans member portal, AveraChart or another patient portal with Avera, your plans and medical information is transitioning to MyChart; however, your login account details are not. You must set up a new MyChart account. Having Avera MyChart enables you to easily see your plan details, pay bills, manage your care and more.
What is the MyChart access code?
The MyChart access code is a code that’s custom to you that enables you to sign up for Avera MyChart in just a couple steps – especially if you have already been a member or patient with Avera or one of our affiliates.
If you’re new here, we may require additional information when you register for an account.
What if I need help with my access code?
For security and privacy reasons, MyChart access codes expire after a certain amount of time. If your access code is lost, expired or you didn’t receive one, try these options:
- Recommended: Contact our help desk at 855-667-9704 for a new code to streamline your registration.
- Contact your clinic and ask them to generate a new code and resend it to you.
- Register for MyChart without an access code. (This requires you to fill out more demographic information than if you were already in our system and had a code.)
What if I forget my username or login ID for MyChart?
On the MyChart login screen, select Forgot login information? anytime if you’ve forgotten your login credentials. You can also go directly to the account recovery page.
