The member portal – online and the app – are your go-to locations for details about your insurance policy, coverage, claims and more. Here are just some of the things you can find in the member portal:

  • Outline of Coverage (OOC): Explains your Medicare benefits, cost sharing information, travel benefits, network hospital restrictions and more
  • Explanation of Benefits (EOB): Provides information about your claims and how they were processed and paid
  • Provider directory: Outlines provider and facility specifics for your plan

Getting Started & Choosing a Provider

See what to expect as you become a new member and what to do once your policy application is approved.

  1. Learn what to expect when you’re a new member – including when to expect your member ID card.
  2. After you have your member ID card, create an account for our secure member portal.
  3. Log into the member portal and view information specific to your plan – including the Outline of Coverage, which explains your benefit details.
  4. Visit the physician of your choice! With Avera Health Plans Medicare Supplement Insurance, there’s no need for a referral for basic care. Learn more about choosing a provider, health care facility and other services.
  5. Be sure to go back to the member portal to review claims including your explanation of benefits (EOB).

With a Medicare Supplement Select plan, enjoy the freedom to see any doctor for primary care services. No referral needed.

Our network includes services for your home health, home medical equipment and hospice needs.


Our provider network includes Avera Health (South Dakota, Minnesota and Iowa locations) plus community network hospitals such as Monument Health, Mercy Medical Center, Brookings Health System and more.


For outpatient surgery or hospital-based care (including any laboratory, imaging or office visits that process billing through the hospital), make sure your provider has admitting privileges at an in-network hospital. If not, ask for a referral to a provider who does.
