Traditional Plans

Get the flexibility to design and create plans that meet the criteria and needs of your business and employees. These plans are great option for groups wanting a lower monthly premium.

  • Offers both medical and pharmacy benefits with preventive benefits covered at 100%
  • Option to personalize benefits, including vision, and Employee Assistance Program (EAP)
  • High Deductible Health Plans (HDHP)
  • Offers both medical and pharmacy benefits once deductible is met with preventive benefits covered at 100%
  • Offers a higher deductible and lower premium than a traditional plan
  • Can be paired with a Health Savings Account (HSA) for tax benefits
  • Puts employee into the driver’s seat of health care

Ultra Plans

  • Offer both medical and pharmacy benefits with preventive benefits covered at 100%
  • One co-pay per office visit, per day for all services provided in the provider’s office
  • Excludes these services: PET scan, MRI, CT scan, SPECT scan, cardiovascular, nuclear medicine, specialty medications and MRA

Preferred Plans

  • Available in Minnehaha, Lincoln and Brown counties in South Dakota
  • Offers both medical and pharmacy benefits with preventive benefits covered at 100%
  • Tiered plan that offers savings when using Avera Network of nearly 2,000 providers
  • $0 co-pays for primary care, urgent care and chiropractic visits
  • No out-of-network coverage
  • Standard Plan option available to be offered alongside Preferred Plans to match benefits for employees living outside the three-county service area

Direct Plans

  • Available in Minnehaha, Lincoln and Brown counties in South Dakota
  • Offers both medical and pharmacy benefits with preventive benefits covered at 100%
  • Network is primarily Avera providers and facilities for cost savings
  • Great for employees who want a more affordable monthly premium
  • No out-of-network coverage

Value-Added Services

The following value-added service are available to you and your employees at no additional cost.

Fitness Center Discount

Membership discounts at select fitness businesses such as GreatLIFE Golf & Fitness Club and Avera McKennan Fitness Center could be available to your employees. 

Virtual Visits

Use it for simple illnesses such as flu or sinus infections. Avera Health Plans Virtual Visits are available to all employees on the insurance plan over the age of 2 and can be used anywhere in the United States. Most plans offer this service with $0 co-pay for visits.1

COBRA Services

Avera Health Plans offers COBRA administration services at no additional fee, if elected. The services provided are compliant with the COBRA/Continuation of Coverage administration requirements.

FSA and HSA Services

Avera Health Plans offers Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA) at no additional fee, if elected. The services provided are through partnership with DAKOTACARE Administrative Services (DAS).
