Avera Health Plans offers flexible plan options to fit your company. Your quote will offer a complete package that can include standard or customized options according to your needs.

Our plan options include the following:

TotalChoice

TotalChoice is a new product for small and large employer groups in South Dakota and select northwest Iowa counties. TotalChoice is our newest PPO plan with the broadest network available, including all of South Dakota's major health systems and many trusted regional providers, along with Avera’s local, community-based providers.

TotalChoice plans also include a travel network, using the UnitedHealthcare Options PPO network for national coverage. Customize coverage by freely mixing and matching existing HMO plans alongside the TotalChoice PPO plan options to give employees maximum freedom and choice of plan types and network options.

Traditional Plans

Get the flexibility to design and create plans that meet the criteria and needs of your business and employees. These plans are great option for groups wanting a lower monthly premium.

  • Offers both medical and pharmacy benefits with preventive benefits covered at 100%
  • Option to personalize benefits, including Vision and Employee Assistance Program (EAP)
  • Design and create plans that meet the criteria and needs of your business and employees

Ultra Plans

  • Offers both medical and pharmacy benefits with preventive benefits covered at 100%
  • One co-pay per office visit, per day for all services provided in the provider’s office
  • Inclusive co-pay. Excludes these services: PET scan, MRI, CT scan, SPECT scan, cardiovascular, nuclear medicine, specialty medications and MRA

High Deductible Health Plans (HDHP)

  • Offers both medical and pharmacy benefits once deductible is met with preventive benefits covered at 100%
  • Offers a higher deductible and lower premium than a traditional plan
  • Can be paired with a Health Savings Account (HSA) for tax benefits
  • Great option for group who wants a lower monthly premium
  • Puts employee into the driver’s seat of health care

DirectConnect Plans

  • DirectConnect plans remain available to eligible employers headquartered in Brown, Lincoln and Minnehaha counties. New for 2026, DirectConnect plans are available to eligible employers headquartered in the following counties: Aurora, Clay, Davison, Douglas, Hanson, Hughes, Hutchinson, McCook, Stanley and Yankton.
  • Offers free1 visits to primary care physicians before having to pay a co-pay, including primary care, chiropractic and physical/occupational/speech therapy visits.
  • Network is primarily Avera providers and facilities for cost savings
  • Great for employees who want a more affordable monthly premium
  • No out-of-network coverage

Value-Added Services

Avera Health Plans offers a variety of value-added services to complement the benefits that group plans provide. Depending on the group plan selected, value-added services may differ.

Value-Added Services for All ACA Large Group Businesses

Your employees receive several value-added services. For example, South Dakota, Iowa and Nebraska ACA large group members receive the following value-added services:

  • $15,000 of life insurance coverage
  • ActiveLife fitness membership reimbursement program
  • Employee Assistance Plan (EAP) at no cost
  • Virtual visits for convenient, 24/7 care
  • Well-being services
  • Coverage of heart screenings through Planet Heart and other participating providers

Fitness Center Discount

Through our ActiveLife program, membership discounts at select fitness businesses, such as GreatLIFE Golf & Fitness Club, could be available to your employees.

Virtual Visits

Avera Health Plans Virtual Visits offer convenient, 24/7 access to care from the comfort of your home. In addition to urgent care for common illnesses like the flu or sinus infections and behavioral health support,2 you can now connect with providers for dermatology consultations and lactation guidance.

Virtual visits are available to all members over the age of 2 and can be used anywhere in the United States.

Urgent care and behavioral health visits are covered benefits for most members. Some limitations apply. Dermatology and lactation consultations are available but may not be covered. Standard visit fees may apply.

COBRA Services

Avera Health Plans offers COBRA administration services at no additional fee, if elected. The services provided are compliant with the COBRA/Continuation of Coverage administration requirements.

FSA and HSA Services

Avera Health Plans offers Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA) at no additional fee, if elected.

Life Insurance

Your employee plan subscribers receive a $15,000 life insurance policy. Additional life insurance options are available for purchase to all qualified groups.

Employee Assistance Program (EAP)

The Avera Health Plans EAP offers confidential counseling sessions with a licensed mental health professional at no cost for employees, spouses and eligible dependents.

EAP can help with issues such as parenting, marital counseling, balancing work and home life, depression, anxiety, grief and more.

The three-visit EAP benefit is included in all South Dakota and Iowa ACA large group plans. All group employees and their family members each have access to a total of three EAP visits every year at no cost. These visits are available even if the employee opts not to enroll in coverage from Avera Health Plans.

All groups have the option to expand access with the purchase of a five-visit benefit option. The five-visit benefit is available to the subscriber, spouses and eligible dependents.

Well-Being Services

Well-being services connect employees with resources to improve or manage their holistic health. Services encourage long-term wellness through engagement opportunities, personalized health plans and education.
